Should you wish to register in person, registration will be held outside the trophy case area
at the Orleans Recreation Complex on Wednesday - September 2, 2009 from 7 PM –
8 PM.

returning players must register by June 30, 2009. Cheques may be post-dated until June
30, 2009. Failure to comply will result in a $50.00 late registration fee.
This does not apply to new players to the OMHA or the Initiation and Juvenile
Programs. Registration is NOT complete

Late Payments and NSF Cheques:
Payment of the appropriate OMHA
registration fee must be received in full before your child will be allowed to
attend Rep ‘B’ tryouts and/or House League player evaluations. Registration
will not be complete and players will not be allowed on the ice until the
OMHA receives: 1) the signed Registration Form; 2) signed Parent and Player
Fair Play Pledge; and 3) the entire registration fee has been paid. NSF cheques will be subject to a $50.00
service charge.

Registration
Information:
·
The
Orleans Minor Hockey Association abides by the GHA "Fair Play"
Policy. Through registration, you and
your child agree to be governed by its rules.
Please sign the attached Player Registration, Parent and Player Fair
Play Pledge, include payment and return to the OMHA. For information regarding
the Fair Play Policy contact the GHA or visit www.gloucesterhockey.ottawa.on.ca.

The
Initiation Program is for 4, 5 and 6-year old players.
·
The
Rep ‘B’ Tryout Fee is $90.00 on or before June 30, 2009. All Rep ‘B’ Levels will have one Minor and
one Major team where applicable and when sufficient registration warrants.
·
Payment
of extra fees could be requested by teams to cover additional costs such as;
tournaments, extra practice ice, etc.
·
The
OMHA reserves the right to move players within a designated Level based on the
player’s assessed evaluation. This
adjustment is made with the player’s best interest in mind.
·
Coaches,
Managers, Trainers and Volunteers are needed to ensure a successful hockey
season. If interested, please let us
know and indicate in what capacity you, as parent or guardian would like to
assist the OMHA. To receive a copy of
the appropriate application form refer to the Forms Section of the OMHA website
or contact the OMHA Office at 834.6553.

The
OMHA Family Registration Plan allows a family with three or more registered
children to deduct $30.00 per child off of each child’s registration fee.

Players
who wish to tryout for the Gloucester Rangers must register for Ranger tryouts
on or before June 30, 2009. The
appropriate OMHA registration fee must be paid in full to the OMHA before
players will be allowed on the ice for Ranger tryouts. For further Ranger tryout and fee information

Waiver:
I acknowledge that
ice hockey is a contact sport and hereby consent to my child participating in
the playing of hockey and agree that my child and I will abide by the Orleans
Minor Hockey Association Constitution, its Rules and Regulations and decisions
of the OMHA Board. I also agree that the
Orleans Minor Hockey Association (OMHA), its officers, agents, volunteers,
directors and its executives shall not be liable to me or my child for any
accident, injury or damage howsoever caused resulting from my child’s
participation in ice hockey. I hereby
discharge the OMHA, its officers, agents, volunteers, directors and its
executives from all actions, claims and demands that I and/or my child may have
for any such injury and damage. I also
acknowledge that the OMHA, its officers, agents, volunteers, directors and its
executives are not liable to me or my child for any injury or damage or loss of
property, howsoever caused, incurred in or about arena facilities, on the ice,
participating in practices, team sanctioned events, hockey games, tournaments
and while travelling to and from hockey events at any time during the term of
the 2009-2010 hockey season, and I hereby discharge the OMHA, its officers,
agents, volunteers, directors and its executives from all actions, claims and
demands for any such injury, damage and/or loss of property.
I further acknowledge and agree that the OMHA does not
ensure and will not be expected, at any time, to ensure the safety or the
proper state of rep air of my child’s equipment used by my child during the 2009-2010
hockey season. I also understand and
agree that my agreement releases and discharges herein, and shall forever
discharge all future actions, suits, demands and other liability and bind my
heirs, legal representatives and assigns and shall inure to the benefit of the
OMHA, its officers, agents, volunteers, directors and its executives and their
successors and assigns.

FAIR PLAY PLEDGE
It is the intention of
this Pledge to promote fair play and respect for all participants within the
Hockey Association and
the
before being allowed to
participate in hockey and must continue to observe the principles of Fair Play.
FAIR PLAY CODE FOR PLAYERS
1. I will play hockey
because I want to, not because others or coaches want me to.

rules of hockey and in the spirit of the Game.

FAIR PLAY CODE FOR PARENTS AND GUARDIANS
1. I will not force my
child to participate in hockey.
2. I will remember that
my child plays hockey for his or her enjoyment, not mine.
3. I will encourage my
child to play by the rules and to resolve conflict without resorting to hostility
or violence.
4. I will teach my child
that doing one’s best is as important as winning so that my child will never
